Client Access to File

If you’re a client, or a guardian of a minor client, of the agency and you’d like to request access to your file, please complete and submit this PIPA form to the RVCS Privacy Officer by emailing us. Email us here.

Note: if you’re a client mandated through the Ministry of Children and Family Development (MCFD), the process is different. Please complete this FOIPPA form and send it to MCFD directly. Mandated clients are persons who must work with a service provider because of a legal requirement or court order as necessitated by MCFD.
